Cotton — the dermatologist default
Cotton is the most commonly recommended fabric for sensitive skin because it is soft, breathable, and well-studied. It absorbs moisture, does not create static, and is unlikely to cause mechanical irritation. The risks are in the processing: conventional cotton is treated with pesticides in growing, and finishing treatments — wrinkle-resistant, anti-shrink — can leave chemical residues. Organic cotton (GOTS certified) or tested-safe cotton (OEKO-TEX Standard 100) addresses both. For people with sensitive skin, cotton basics in organic or certified varieties are a reliable starting point.
Merino wool — not what you would expect
Many people with sensitive skin avoid wool on the assumption it will itch. Fine merino wool at 18.5 microns or below is soft enough for most people because the fine fiber bends when it contacts skin rather than creating mechanical prickling. People who have reacted to standard wool should try fine merino before assuming they cannot wear it. Merino is also naturally temperature-regulating and wicks moisture — both beneficial for skin that reacts to heat and dampness.
Silk — naturally hypoallergenic
Silk has a protein structure similar to human skin, which is why it rarely causes reactions. It is smooth against skin, reducing friction in areas prone to irritation. It is temperature-regulating in both directions — warm in cool air, cool in warm air. True silk allergy is uncommon; most reactions attributed to silk are actually from dyes or finishing chemicals in the fabric. For people who need the most skin-safe fabric available, silk is the strongest option.
Alpaca — lanolin-free alternative to wool
Standard wool contains lanolin, a natural wax that some people react to. Alpaca fiber contains no lanolin, making it a useful alternative for people who have confirmed lanolin sensitivity. Alpaca is also very soft — fine alpaca is comparable to cashmere — and warmer by weight than wool. Baby alpaca (the first shearing) is the softest and most suitable for direct skin contact.
What to avoid
Formaldehyde-based wrinkle-resistant finishes are a frequent skin irritant found in otherwise natural fiber garments. Azo dyes — widely used in textile dyeing — are restricted in some countries due to skin and health concerns. Polyester blends added to natural fiber garments for stretch or durability trap heat and moisture in a way that pure natural fibers do not.
